Background

Calvert County Government's current agreement with CareFirst for employee medical, prescription, dental and vision benefits is scheduled for renewal effective July 1, 2026, through June 30, 2027. The renewal continues the county’s existing benefit structure, carrier relationships and contractual arrangement previously approved by the Board. No changes to plan offerings were made as part of this renewal cycle.

Discussion

The Fiscal Year (FY) 2027 renewal proposal maintains the county’s current self-insured medical and prescription coverage structure, as well as dental and vision benefits administered through CareFirst. While the overall program structure remains unchanged, the renewal reflects updated projected claims costs, stop-loss premiums and administrative fees for FY 2027 under the terms of the existing multi-year agreement. The renewal also continues the administrative fee guarantee through June 30, 2029, along with the annual $75,000 wellness fund and $15,000 communications fund. Although the accompanying document is titled “Firm Proposal,” it serves as the final renewal document for Fiscal Year 2027. The term “proposal” is used because final program costs depend on actual claims experience and cannot be determined at this stage. No further revisions or negotiations are expected beyond standard claims-based cost adjustments inherent in a self-insured program. The projected total annual cost of the program for FY 2027 is approximately $24.6 million, which has been incorporated into the approved FY 2027 budget and previously reviewed with the Board during budget work sessions.
